Premier Care Coordination is launching a new software care coordination platform called Juun in September.
The platform was conceived to help physician practices and health organizations navigate four Medicare care coordination programs:
- Community Health Integration
- Principal Illness Navigation
- Chronic Care Management
- Transitional Care Management
The software launch will give caregivers a second way to use Premier's expertise to manage the Centers for Medicare & Medicaid Services programs.
“I spent some years in trauma intensive care watching patients come back for problems that were preventable between visits,” said Rachel Yates, founder and chief executive officer of Premier Care Coordination, in a press release. “Medicare already pays for the care that keeps those patient’s home. The gap is capacity, not willingness. We built Juun so a practice can deliver this care with the team it already has.”
Yates spent almost a decade as a clinical nurse and found that Medicare patients are served best when they have an advocate. That experience led her to found Gary-based Premier Care Coordination, which virtually supports patients and their caregivers.
“Northwest Indiana has a Medicare population that needs more between-visit support than most practices have the staff to provide,” Yates said. “Premier Care Coordination was built to close that gap.”
Her company partners with primary care offices that accept Medicare patients with chronic and serious illnesses. She helps patients find often unused funding introduced by the Centers for Medicare and Medicaid Services in 2024.
