Valparaiso-based HealthLinc will take over operations of Northwest Medical Group – Pediatric Care on Dec. 1.
The practice has two locations in La Porte and Michigan City.
“We are looking forward to working with these highly regarded providers from Northwest Medical Group and serving even more children in La Porte County,” said Melissa Mitchell, HealthLinc CEO, in a press release. “We are committed to ensuring the current Northwest Medical Group – Pediatric Care patients experience a smooth transition to HealthLinc.”
The La Porte office will become HealthLinc La Porte Pediatrics at 1509 State St. It is next to the HealthLinc clinic at 400 Teegarden St. The two offices will work together to provide expanded services, including dental and behavioral health care, community health workers, clinical pharmacists and health insurance assistance.
“This will be a unique opportunity for the pediatric providers at Northwest Medical Group to augment their high-quality care and service with additional support for their patients,” said Simon Ratliff, chief executive officer for Northwest Health – La Porte. “As part of HealthLinc, their patients will have ready access to community health workers, behavioral health clinicians and dentists.”
The Michigan City office of Northwest Medical Group will relocate to the HealthLinc Michigan City at 200 Alfred St.
HealthLinc assures current patients and staff that the transition will be smooth and easy. Doctors at the La Porte location include Usama Moustafa and Hadeily Salazar.
“We will continue to work closely with Dr. Moustafa, Dr. Salazar and their nurse practitioners as they transition to the HealthLinc umbrella of services,” Ratliff said.
HealthLinc Inc. is a not-for-profit Federally Qualified Health Center. It was founded in 1996 as Hilltop Neighborhood House in Valparaiso. It now has 13 clinics that serve East Chicago, Knox, La Porte, Michigan City, Mishawaka, South Bend and Valparaiso. It also offers a mobile health center.
