Franciscan Health Rensselaer is pleased to announce Nicole Krol, R.N., has been appointed as diabetes educator.
Nicole Krol is now accepting appointments for diabetes self-management education based on a referral from a healthcare provider. Patients will meet one-on-one with Nicole at the Franciscan Health Rensselaer, 1104 E. Grace Street in Rensselaer.
Managing diabetes well reduces the risk of diabetic-related health problems. Patients will learn more about treatment plans including the importance of healthy eating, monitoring blood sugar, taking medication and physical activity. A summary of self-management will be sent to healthcare providers after patients visit.
Education, like medication, is part of a treatment plan for those referred to diabetes self-management education. Learning everything they can and following patient’s specific plan will maintain good health, over a lifetime with diabetes.
Diabetes is a common chronic disease which may result in a variety of serious health conditions. The highly trained education staff at Franciscan Health Rensselaer offers compassionate support and encouragement while helping patients successfully manage their diabetes.
